Intake Form
In order to provide you with the most comfortable and beneficial massage, I have an intake form for you to fill out before or upon arrival to your first appointment. If possible, please download it here, print and bring to your appointment or arrive 5 minutes early to complete it.
Cancellation
I understand that unanticipated events happen occasionally in everyone’s life. Business meetings, project deadlines, flight delays, car problems and illness are just a few reasons why one might consider canceling an appointment. In my desire to be effective and fair to all of our clients, I have adopted the following policies:
• 24 hour advance notice is required when canceling an appointment. This allows the opportunity for someone else to schedule an appointment.
• Appointments canceled within 24 hours will be charged $25.
• Anyone who either forgets or consciously chooses to forgo their appointment will be considered a “no-show”. They will be charged for their “missed” appointment and their balance must be paid in full before receiving future treatment.
Arriving late
If you arrive late your session may be shortened in order to accommodate others whose appointments follow yours. Regardless of the length of the treatment actually given, you will be responsible for the “full” session. Out of respect and consideration, please plan accordingly and be on time.
Massage Termination
Massage will be terminated immediately if a client makes any sexual advances or requests. Even questionable sexual discussion can be cause for termination.
A session will not be conducted if the client is intoxicated, using drugs or threatening my safety, the safety of others in the building or themselves.
When a massage is terminated for one of these reasons, full payment is due.
